Great! Your protocol has been approved. Now what? There are some ongoing responsibilities for each Principle Investigator once your protocol has been approved.Ìý
Major Time Point One:
Two months prior to the first anniversary of your protocol, you must submit an continuing review through eProtocol. This simple form updates the IACUC Office on your animal use for the past year, and gives us an idea of your planned use for the next year. Note: It is the PI's responsibility for tracking the approval periods (anniversary dates) of the protocol and submitting the review in a timely manner.
Major Time Point Two:
Two months prior to the second anniversary of your protocol, you must submit another continuing review through eProtocol. This simple form updates the IACUC Office on your animal use for the past year, and gives us an idea of your planned use for the final year of your protocol. Note: It is the PI's responsibility for tracking the approval periods (anniversary dates) of the protocol and submitting the review in a timely manner. Ìý
Major Time Point Three:
At least three months prior to the expiration date of your protocol you must submit a continuing review through eProtocol.ÌýSelect "Start Continuing review". Please update all areas of the protocol to reflect current procedures (i.e. remove any personnel and/or procedures that are no longer in place). Note: It is the PI's responsibility for tracking the approval periods (expiration dates) of the protocol and submitting the renewal in a timely manner.
Continuous:
Report all unanticipated/adverse eventsÌýwithin 72 hoursÌýby submit the Incident Report Form via eProtocol system to the IACUC office that occur on your protocol. The investigator must notify the IACUC Office (303-492-4787) of any unexpected results that adversely affect the welfare of the animals. Investigator must report any unanticipated pain or distress, morbidity or mortality of animals to the attending veterinarian and the IACUC. If there is an adverse event involving the health and/or safety of any researcher or animal handling personnel, the investigator must contact the IACUC office (303-492-4787) and EH&S (303 492-7072),Ìý and complete a University Risk Management Employee's Injury Report Form within 48 hours of the event.Ìý
