To better manage change requests for staff throughout the college, CEAS HR requests that supervisors and/or human resources liaisons (HRLs) formally request changes by submitting a Change Request Form.

Change Request Forms should be initiated by a supervisor or HRL on behalf of a staff employee within the College of Engineering & Applied Science. They can be requested for the following scenarios:

  • Requesting a staff promotion
  • Requesting a salary adjustment for a staff employee (without promotion)
  • Requesting a new staff position (i.e. new headcount)
  • Requesting to backfill a vacant staff position
  • Requesting a new supervisor for a staff employee

By having supervisors or HRLs request approval for these HR functions utilizing this Change Request Form, we are creating a system where all requests go through a standardized approval process, and requests are reviewed and approved equitably through a data-driven process.

To start the form, please identify your role as initiator of the change request (options include either the staff employee’s supervisor or HRL). Then, provide the name and contact email address for the supervisor and “next-level” manager or department chair. Verify that the information is correct before submitting.

Visio org charts

A DocuSign form will be created on your behalf. You may be asked to verify your identity by providing an access code that is sent directly to your email address. Please complete the form in its entirety, providing as much detail as you can pertaining to the request, especially the rationale for the request.  All required fields must be completed before submitting the form. Please remember to include your draft Visio org chart (from Microsoft 365) in the attachments and consider providing context to the optional fields as well. 

Once all pertinent information has been submitted, the supervisor of the employee will be prompted to sign, then the “next-level” manager/dept. head, then CEAS HR and Finance approve the request. CEAS HR will reach out to the form’s initiator to provide more information about next steps.
